CONTRACTED MAP MEMBERSHIPS
Applicable Memberships: *QB Annual Membership (MAP) / *QB 6-Month MAP Membership
Membership Terms & Conditions: 12 month commitment, eligible for up to 3 months of suspension with valid doctors note (1 month of suspension for 6-Month MAP) , cannot cancel unless member moves 25+ miles away from a BKB facility and provides valid proof, monthly payments are non-refundable; must request to cancel membership online through our Zendesk support portal and receive confirmation that the request was submitted successfully at least 15 days before their next billing cycle, otherwise, member will be charged 1 more month before their cancelation goes into effect
ANNUAL PIF MEMBERSHIPS
Applicable Memberships: *QB Annual Membership (PIF)
Membership Terms & Conditions: 12 month commitment, +1 free month for a total of 13 months when paid in full; eligible for up to 3 months of suspension with valid doctor's note; non-refundable
MONTH TO MONTH MEMBERSHIPS
Applicable Memberships: *QB Month-to-Month / *QB Student Month-to-Month / Any Sliding Scale Membership
Membership Terms & Conditions: No minimum commitment; not eligible for suspensions/freezes; must request to cancel membership online through our Zendesk support portal and receive confirmation that the request was submitted successfully at least 15 days before their next billing cycle, otherwise, member will be charged 1 more month before their cancelation goes into effect
WEEK TO WEEK MEMBERSHIPS
Applicable Memberships: *QB Week-to-Week / *QB Student Week-to-Week
Membership Terms & Conditions: 2-week minimum commitment; not eligible for suspensions/freezes; must request to cancel membership online through our Zendesk support portal and receive confirmation that the request was submitted successfully at least at least 6 days before their next billing cycle, otherwise, member will be charged 1 more week before their cancelation goes into effect; membership is billed weekly on Fridays

I understand that should I want to cancel my membership, I must request to cancel online via the BKB Support form which can be found on the Brooklyn Boulders website: https://brooklynboulders.zendesk.com/hc/en-us/requests/new. Cancellation requests made in-person, over the phone or via email will not be considered.
I understand that my request has only been recieved when I recieve a confirmation email from Zendesk, stating the date and time of my original submission, and that I may be asked for proof of this receipt. I understand that if I do not recieve a confirmation email, it is assumed that my cancellation request was not successfully submitted and therefore, BKB has no record of my request.
I understand that I must submit my request at least 15 days prior to my billing date or I will be charged for one more month of membership before my cancellation goes into effect.
I agree to not dispute any BKB membership charges that are made within the parameters outlined in the Cancellation Policy Agreement and Membership Terms & Conditions.
I have read and understand the cancellation policy as stated above.
